Extracurricular Scholarship
DSFT believes that every individual with Down Syndrome should have the chance to reach their full potential and be involved in the community. To aid in inclusion and to foster a sense of belonging, DSFT is pleased to offer an extracurricular scholarship program. From ballet to soccer, art classes to cooking classes, this scholarship will help to offset the cost of any inclusive activity (outside of therapy and tutoring) that an individual with Down Syndrome might like to participate in. This scholarship can be applied for once per year.
Extracurricular Application Requirements
Purpose:
The purpose of this application is to allow individuals with Down Syndrome to request assistance with access to creative outlets that foster community connection, growth, learning, friendship, and understanding. The scholarship can be used for music, sports, art, or any other activity of interest, but should not be used for tutoring or therapies. Individuals may apply for this scholarship once per year.
1. Eligibility Criteria

Diagnosis of Down Syndrome: The applicant must have a documented diagnosis of Down syndrome.

Residency: Applicants must be a resident of Tallahassee, FL.

Name of the organization, team, or class with whom the applicant will be participating.
2. Application Form

A completed scholarship application form must be filled out online.
3. Personal Statement or Essay

A brief essay or personal statement from the applicant or a parent/guardian. This might describe the applicant's interest or passion for an activity and why the applicant would like to participate this activity. [Photos of the individual participating in this activity would be appreciated.]

Reflection on how participating in an inclusive activity in the community can help change perceptions about individuals with Down Syndrome.
4. Parental or Guardian Information

For younger students, the contact details from the parent or guardian are requested.
