Therapy Scholarship
Many individuals with Down Syndrome benefit from therapy to assist in their development. These therapies can take a variety of forms, such as physical, speech, and occupational therapy. DSFT knows that these services can be expensive and sometimes, even with insurance, the cost can be overwhelming. We are pleased to offer a therapy scholarship to help make these costs more manageable. The therapy scholarship can be applied for three times per year.
Therapy Application Requirements
Purpose:
Our goal is to provide financial assistance to individuals in need of physical, occupational, speech, music, art, or other types of therapy who are unable to afford the full cost of care. Please complete the application on our website to help us better understand your situation and how we can assist you.
1. Eligibility Criteria

Diagnosis of Down Syndrome: The applicant must have a documented diagnosis of Down Syndrome.

Residency: Applicants must be a resident of Tallahassee, FL.

Name of the treatment center or licensed therapist who will be working with the applicant.
2. Application Form

A completed scholarship application form must be filled out online.
3. Financial Need

Statement describing the need for financial assistance.

Health Insurance information and co-pays per session.
4. Therapy Plan

Applicants need to submit a brief plan documenting the type and course of therapy that will be conducted and how many sessions are anticipated.

Applicants should estimate the total therapy cost per session that will be paid out of pocket.
5. Deadline and Submission Instructions

The applications must be received by December 15th to be considered for the January semester.
